Sounds like about voodoo, some believe its about drug - "...The pins and needles prick the skin of little dolls..." & "...The little doll is you yeah!..."

Ive always thought this song is about "God" or whatever rules our world. We are all "Little Dolls", unable to escape from the pain of living, and ultimately death. Some of Ozzys lyrics(most of them he didnt write) in 70's to 80's have a "satanic" bent. like sabs "who are you?" in this view God is an impersonal force, that could care less about our suffering. anyways in this song, God is a killer, that remains a mystery. "the worst is yet to come" could be a allusion to going to hell, after death.

It also could be a song about a serial killer/torturer, but im less partial to this interpretation.

The meaning of the song so obvious that people are trying to find some hidden meaning other than the meaning that is staring right at them. It s about being under a voodoo doll curse.. that's it.. nothing more.. no drug addiction symbolism.. no higher religious symbolism.. It's about someone in a state of living hell because someone has a little doll of him and is sticking pins and needles into it.. Seriously, sometimes a apple is just and apple.. LOL

I find the meaning straightforward when he sings "The little doll is you." The song is basically saying that we are our own worst enemy and don't realize it. The killer is a mystery....but all the while the killer is ourselves...
