So... she wrote this song because a woman of her family has alzheimer... Really sad song.

It think she's praying to God, but she thinks He isn't even listening to her.

She's worried because she knows this woman is suffering and slowly dying.

"I miss her even though she's still here"
This woman's still alive, but Jessie misses the way she was before having alzheimer.

"You need to listen/ Don't let her disappear"
She's begging to God that this women won't forget everything.

"I'll give her my forever/ If it meant for a day she could really be okay/ 'Cause I miss her/ Even though she's still here"
She would give her whole life just to see this woman's alright just for one day.

"Is this pretend?/ Will she really not know my face?"
She wonders if this woman is pretending or if she actually can't remember Jessie.

She asks God for a cure... The woman doesn't even know she's ill.

This is the only part of the song which I don't understand... Maybe 13 is the woman's favourite number and green is her favourite colour.

"Promise me this/ That she knows/ That the people that she loves/ Will love her in the skies above"
Jessie wants God to promise her that when the woman dies, she'll know that her family and friends love her, even though she can't remember anyone.

I think this song is about an ex-boyfriend who she still loves. It was a long distance relationship that ended because the boy "gave up on them in the end" (because of the distance). The whole chorus are the "words I (she) wished you (he) said".
