Well, I think it mainly refers to a broken relationship. Which they both knew, at one point, to be true (love). But for one reason or another things fell apart. He still loves her although she's moved on to someone new. The "sugar pill" refers to the fake relationship she is in now, not true love but based on primitive desires. He can't let go of the love they once had, no relationship can live up to the happiness and beauty he felt with her.

Not sure why there is such an insestrial theme flowing in here, kind of weird haha.

My interpretation.. A prophetic love story. Two lovers are falling apart and she sees it but he cannot let go. She is in misery because the love they once had is no longer present. "The Mother We Share" represents Mother Earth, with her love and compassion will not let his proud head from falling, as in dying. The loss of his first/true love will hurt but will eventually open his eyes to his old ways and lead him to change for the better. She will keep his head as a souvenir because while they are separated he will be lost, mentally. His love will forever lay with her and her absence will make him strive to be better.
