I always figured that shaking paper referred to omamori, the japanese paper charms that people would hang for protection from demons or other evils. And I figured she was calling the person that verse is directed to a demon by saying they were also scared of the sound of shaking paper.

Although this may or may not be unrelated, sometime around the making of this album Chan got an abortion (the subject of Nude as the News). So, I always assumed that was what, at least partially, this song was about. Also possibly about premarital sex/relationships in general.

I think the beginning is right after some sort of drowning accident. She's being dragged out of the river, she describes the man who helped her as nice... but her lover's been taken away. The rest of the song is more recent events, events that cause her to become more more deeply depressed and manic.

I think the last stanza, the parallelism is referring to her own desire to be at the bottom of the river. Suicide.

Cat Power – Red Apples (Smog cover) Lyrics 14 years ago
So here's my two bits on this beautiful song.

I think the red apple is supposed to be like temptation. She is going to meet this widowed woman at the river to comfort her, and the widow gives her and apple as a symbol of temptation. So they're having a relationship.
I don't know what the black arms mean. I think she is sleeping in her arms while they are together, but maybe the singer doesn't feel like she loves this person. Maybe the widow just needed comfort, needed someone to love. And the singer was there to provide but didn't reciprocate.
I don't know what the simile of the husband as a horse means, other than to lead into the cart imagery. I think the millions of red apples is a symbol of the husband either moving on and being okay with the widow's new relationship, or is mocking the singer who is now in a relationship she has no feelings for.

I know this is a cover song, and I believe the original singer was a male. I think the fact that Chan is female adds another layer of "sin" to this song, which is ultimately what I think it's about. A woman losing her husband, loving another woman after his death, and being haunted by her husband for it.
