When I hear this song, I think of the aftermath of a bioterrorism event. It's from the POV of a survivor, witnessing the destruction and dealing with the intrinsic terror that seeing so many dead would cause. There's fear, grief, disbelief, and in the midst of it all, a strange beauty in being spared such a grim fate.

I think a good interpretation lies in a combination of several things that have been pointed out by some of you. I think the first verse is describing antisocial or self-isolating behavior, and as the following chorus assists in suggesting; this is a product of paranoia. "Into my head it goes" or "until my headache goes" can both describe the resulting perpetual headache he gets from all the psychological strife.

The next verse describes the drugs he uses to alleviate his pain, both physical and mental. It tastes good in that it's "just what the doctor ordered" as he knew it would be.

The last verse could be describing regret, or just further paranoia produced by the drugs. I'm not positive on it.

I have to agree with E-Double, and to elaborate on that just by saying that "two smiles is a long walk" probably means, in that context, that it takes a lot of time and effort to reach mutual happiness in the relationship.

a music video for this song would definately feature some kind of old fashioned, grainy black-and-white-filmed circus featuring a top-hat wearing Damon Albarn conductor leading the musical intro to this song. Just listen to the rhythmn there. Of course, as with the music, the video would feature "three changes" and transition the video's mood/motif.

It's actually Beck's message to his son, or actually either of his children (he's expecting his second). Musically it's just like a lullaby, but the lyrics also point this out. For instance;

You're smaller than a stone
Bigger than a road
Farther than an ocean
Closer than a soul
Every little word
And every little step
Every new direction
Closer you will get

The end seems like it could also be a message to his kid(s) as they get older:

When you bring it down
All the false sides
When you make him yours
Don't forget he's yours

Although it makes references to nurses/maids, I think that's just being used as a metaphor to describe being betrayed by one's mother as a child. The first verse especially suggests that there is a certain security about the relationship which turns out to be a false sense of security.
'Course, it could actually be a nurse...

In the first verse, I think he's reacting to still playing the old blues-rock that he wrote years ago, while the things that inspired him to write those songs are no longer what is relevant to him. I.e. "I'm bringing back ghosts that are no longer there..." Furthermore, there's sort of a different appoach to writing personal songs when your setting and lifestyle have changed as they have for Jack over the years.

The song goes on to, what seems to be, references regarding his wife. She's a red-head, a trait which Jack obviously holds a deep attraction for. Several lines could be referring to her, their relationship, or some of the things they might be struggling with. Though, that part is heavily speculative on my part...

The third theme is the existential tone the song takes on. The last lines are the most interesting bits... Perhaps Jack realizes that music is not only what is best for him, but it's paying the bills, though he also recognizes that the money will be irrelevant when he's dead. Maybe he is wondering if his songs are accomplishing nothing. Case in point:

"Well sooner or later the ground's gonna be holdin' all
of my ashes too
But I can't help but wonder if after I'm gone will I still have these three hundred mile per hour, finger breaking, no answers makin', battered dirty hands, bee stung and busted up, empty cup torrential outpour blues

One thing's for sure: in that graveyard
I'm gonna have the shiniest pair of shoes"

It's like he wants his music to exercise his demons, but all it's accomplishing is making money, which is unimportant compared to the formerly stated desire.

Jesus, that came out all wrong. Now that I've got clarity of mind let me say what I meant:
First, I really like this song. One of my favorites on the album, and I've had an experience much similar to the one described in it.
When I said "downgrade", I just meant it was a little more simplistic in composition than other songs on the album (not meaning minimalistic).
What I was ultimately getting at was that the song is in-character and kind of a representation of "angsty" modern rock, though that is by no means meant to be a negative thing.

Doesn't something seem a little off here? I think it's a good song, I like it a lot (and can identify with it, too) but hold this song up stylistically with any other White Stripes song to date: it's a little off. Not to say inferior, but even when compared to others on the album, definately a departure in songwriting for Jack. Though, in this case it seems to be a downgrade...intentionally.
Take the theme of the song first: a shy guy who meets a 16 year-old and falls in love. But, because of his low self esteem ultimately shuts her out. ?Angst?
Now, take the writing of the lyrics themselves. Not Jack's usual tone or anything. We can assume it's an in-character song, not Jack's own personal experiences.
Now, thirdly (and please stick with me here, I'm tired lol): take the song's very format. A nice, intermidiate sounding acoustic progression that is fingerpicked. Soothing, sad vibe. Then the chorus kicks in, and it's an uplifting cacophany of guitar and synth (?) chords and an uplifting change of tone that goes from murmering (verses) to strong, hook-laden singing (choruses). This very format defies Jack White in my opinion, in it's presentation here. It's too plain. It doesn't have the right vibe.
So, I think the music is kind of spoofing the modern rock that's becoming popular now in it's style and composition. It might just be me, but the pieces fit... Besides, I heard that Rag & Bone and I'm Slowly Turning Into You were both exercises in utilizing Meg's voice in different and new ways. I know she's kinda sang n' spoken in verses before and such, but her roles with her backing vocals seem like new (possibly exploratory?) territory in this album. I think if this is pursued, they may use her voice & production values to make effects in the song, like another instrument.

It's about mental insanity. There's little real meaning to the lyrics themselves, but through the tone of his singing, Damon Albarn communicates insanity and a sense of paranoia which leads to aggression or full on dementia from verse to verse: "To kill the model from my door" being the peak of the dementia. I'm not saying i'm a psychologist or know anything about psychology, but I make this observation as it fits my own parallel experience with insanity.

Back when people were sentenced to hang for their crimes, at times people would wait behind the gallows until the trap doors were dropped, at which point they would grab the legs of those being hung and pulled down on them, to ensure a quicker and less excrutiating death. These were the "hangers-on" I'm afraid. This song is a surreal description of an execution in the Great Britain of yore.
Though your interpretation is admirable, screamas.

This song is about touring, without a doubt since Beck said it himself. It's about having to follow a schedule that determines where you will be and when for months on end, and although it is a machine-like regimen, he still wants to maintain a human connection with the people he meets and to his fans. Readymade = something that can be deployed at anytime, like an automaton.

It might not be directly referencing his sister's suicide, but that's what I think about when I hear this song. It's a powerful sentiment regarding loss of innocence and the loss of a loved one.

Think about this song in that it's him as a child and his sister. Especially the last verse:

I'll pick some daisies
From the flower bed
Of the Galaxy Theater
While you clear your head
I thought some daisies
Might cheer you up

He's just trying to make his sister feel better. An innocent young child could not understand the serious depression his sister's going through, and can't comprehend that she might commit suicide, which she later does.

Thinking about it that way makes me cry, it's so powerful. It may not be accurate in accordance to E's life, but it works nonetheless.

Hmm...I like Buck's analysis of the female singer's interlude, moreso than mine, actually.

I still believe it's about the war, but considering the sex/drugs theory, I would say that if that were true, the song describes the roller-coaster ride turmoil that drugs and loveless sex (one night stands) entail. That's mostly the rapping part; the pleasure, pain, and excitement of it all. Then, however, when you are through with your indulgence you are left all alone, and life has little meaning; "When the morning comes it doesn't seem to say an awful lot to me..."

I guess I could live with either explaination.

A few corrections - it's "Sitting at the edge of the sea" and "Shifting through the mud and the bricks". I guess I could be wrong about the second line but that first correction is definately right.
And I don't believe that Meg was on this album at all, no. I must say I really like this band though.

I think it's about when things get out of hand, things are said and taken the wrong way, and before you know it you're at ends with your best friends. Then you realize that you acted foolishly and almost threw away a good thing. Step back, relax, life will get better. Just don't call it quits because the friendship is all you have left.

There have been some good interpretations so far, but I don't think this is about sex, or masturbation. I think it's either about the war in Iraq, or possibly the israelis like alsoa said. Here's my interpretation, being from the perspective of a soldier in Iraq.

The "All Alone"s could be a number of things, possibly feeling alone and lonely, seperated from a loved one who's back home. I haven't figured it out completely yet.
"skin of my tooth like seat of my boot like" - really good lines. It's comparing the skin of his teeth (which is incredibly thin) to the bottom of his boots. In other words, he's treading on thin ground. The next step he takes could be his last.
"fly in my soup like where's the waitress?" - It's a bad situation, a big mess, and there's no one around to fix it.
"can i take this, really, can i finish this? these years and all these creatures it's my mistake, i'll make it" - having doubts about the war, knowing it's probably wrong, but going through with his duty anyway.
"shakin' all them bangs out chemical cutthroats bound to blow the brain out cut to the brain" - probably just generic things about fighting and killing as a soldier. The next lines like "show no shame" and "twist this blade as plain as day" are just about doing his duty without regrets. You can probably see how the other lines fit.
Martina Topley Bird's verse is probably imagining a lover who is back home, and a time where the fighting will be over and there will be peace.
