i took "i just might die with a smile on my face" as being that at the moment that he finally "drove the point home" (double entendre or not) he realises that it is sort of funny. that he was taking everything so heavy and now hes feeling a release of all of that, and its so much better than what he had felt before. that he would die at peace.

and I'm welcome no more" then i was sort of leaning towards it being about how hes like being kicked out of his house because no one accepts him, and hes with this boy hes in love with telling him that they should run away together, and like hes to shy to say that outright, so this is all hes thinking in his head. about like, dying with this boy would be heavenly and all...

no i totally think this song is just outrageously sarcastic. yeah, the last lines aren't so biting, but you cant deny the sardonic undertones of the rest of the song.
