Well, I've heard from many sources that this whole album is based on people he met while in rehab (back then rehab wasn't what it is today, they basically threw him in an asylum with psychotics and mental patients). So, Millie and Billie is about a murderous couple that was there, Nurse Rosetta is about a priest who, well, let's just say can't keep it down, etc etc. All supposedly based on real people. And this one, I believe, is about himself. I'd say that's pretty big of him to tell us all about the crazies in the asylum, but before he does so admit that he also was one and tell us about himself.

I don't know why, but this song makes me think of baseball...pretty weird, I know...

What Surgicalsteel said is correct with the Title Song, From The Inside and maybe one or two others about himself. Always thought How You Gonna See Me Now" was a great song and Same for "Quiet Room" which are the two others that are about himself.
