The song is based on a Nightmare Alice had, in which he killed his wife.
Actually the entire album is based on it.
Hence we have "Welcome to My Nightmare"
Okay so Steven passed out, and then woke up in the basement. He's looking for his wife. Instead, what he finds are a bunch of bloodstains. You guessed it. They lead to Steven's diarrhea explosion in the tub, sink, and toilet. He feels like a man because he didn't realize that much poop could come out of him.
