...Much like "Cold Ethyl" in the Refrigerator, this song is about Alcohol. Seeing "Red" is Wine...a "Blue Lady" is a Cocktail (and also, brilliant foreshadowing for "Cold Ethyl").

I agree with all the comments, but nobody's looking at the lyrics. So - a really catchy, jaunty little ditty that gets your feet tapping & your fingers snapping & is about extreme domestic violence! Love it. Only Alice could get away with it.
I'm afraid morbid morag is wrong. This song is about poops of different colors. The man is intrigued with the oddly shaped and colored poops so he looks down and names them all things like "blue lady" and "red." He also loves to feel pain.
This song is about drug addition/withdrawal. The "blue lady" is a a synthetic heroin. It is pretty self explanatory if you understand the drug references.
"love to see red" is NOT "get angry as in common slang but refers to the way blood is pulled into a syringe before injecting heroin, blue lady is synthetic heroin, and the skin crawl references are the withdrawal symptoms (heroin addicts have been know to scratch off their skin in withdrawal possible because of hallucinations)
