I feel like there might be some biblical reference in there somewhere, but I'm not sure.
If not, then I think it's just a song about a guy with schizophrenia whose true morbid nature comes out at night to slaughter people. During the day, he looks like an ordinary man, or loner, that everyone seems interested in; yet no one has a clue of who he really is or what "terrible secret" he's hiding.
The next morning after he's done his killing, remnants of the crime are seen everywhere, yet he stays seated with an innocent yet wicked smile on his face.
Moral of the story: get to know someone well before they end up chopping your head off with no regrget.
