It's about the situation in Northern Ireland. On the one side hand you've got the British government trying to assure everyone that things will be just fine and everyone will be perfectly happy - while they do nothing to stop the violence between the loyalist and republican terrorist factions. And, of course, everyone else is caught in the middle - hence "domestic refugees" who "sink in the same boat as me". Basically, everyone's screwed.

I'm not very good with abbreviations... Can someone explain what a "head O.D." is ?

Pretty sure it just means "Over Dosed" as in a drug overdose though in this context I think its referring to be "dosed" with a belief. The preceding lines are

"Love is for the weak Or so you'd have me believe The thought's killing me"

Also I think what the lyric should be read as "a head o.d'ed."

Took me a minute to figure out how the phrase fitted into the lyrics but I think that's a pretty decent approximation.

I had never thought it dealt with a specific context or specific country, but what skroo says makes sense. I've always heard this song as a universal expression of... Being screwed by authority and governements and feeling helpless.
