This is another Shakespeare reference, this time to Macbeth, and Lady Macbeth's attempts to clean her hands of what she imagined to be King Duncan's blood. This is the origin of sayings like 'Bush and Blair have blood on their hands'.
This applies to the rich generally, hence 'silver spoons' and 'how they other half die' instead of 'how the other half live'. Of course it's something like 95%/5% but it's only a saying!
I always loved this song on "Heartwork". Especially the lines "Out damn spot out!" and "Bloody hands never wash clean." This song is about murder, no question, and that a person guilty of murder can never be let off the hook, or shouldn't! This album was a complete 180 degree turn around for the band, where the subjects really don't go into the intricate subject of forensic pathology, like on their past albums. Having said that, it's obvious that the subject of death is definitely there, as it should be in a death metal album.
