One of my most favorite lines in all of his songs. I think this is specifically about how he likes a girl and how it will get to a point where he will just have to tell her.

No, he's actually singing about a cigarette, and how he people didn't use to realize how bad they were for you. The song is basically just a list of things that start out great but can come back to get you later: gluttony, smoking, infatuation, old age.
