I think Malkmus is referring to the fact that some might see him as cold hearted, or a pig when it comes to women. Anne is a girl who is sad ("don't you cry") because of this, but Malkmus retorts "Don't believe in what they say about me." The first verse would be sung from Anne's perspective. The second is where he explains himself, "My art is not a wide open thing" meaning it's not for everyone and people are going to get him confused.
Someone once told me they thought this song was a swan song of sorts, where Malkmus is explaining he does not want to be in the band anymore, and tellings FANS dont you cry.

This part of the song is my favorite part of any pavement song.
Malkmus always sings about many thing at same time. This song is about a girl named Ann, but it's about the end of Pavement too.

He mentions a hospital...and peole above taking us out....and 'you never had a chance,' so is this a song to a dying lady named ann....or, did someone ann knows die--and now she's crying....it could be anything with Malkmus....
What does the whole "hospital man" line mean?

I believe that Ann is suicidal. When youre suicidal they put you in a wing of the hospital called the psych ward.
Everything there is designed so that there is no way you could possibly commit suicide, including being checked on by staff every 15 minutes- hence the "rooms to live in, but no room to give in" line that comes immediately after the one about the hospital man.
