This song is about suffering from an eating disorder, most likely anorexia nervosa.

"I had a hole in my heart" meaning there was something empty inside. Most people struggling with eating disorders have deeper issues than wanting to be skinny. The hole in her heart represents the feeling that fuels the eating disorder, example maybe she experienced a tramatic event and does not feel whole anymore.

When she says "so i threw away my plate, cause nothing would fill me up, whatever i ate" She means that her way of coping with the stress and the hole in her heart is by self distruction, starving herself. Food would not help her fill the hole in her heart, eating food ment she had no control, so she threw away her plate to have spome control over her life.

"i'm hungry" Meaning shes hungry for some kind of love and acceptance she feels she is missing out on.
