I would assume the spoon refers to the spoon used to dissolve heroin, but the lyric "into the blood" is multiply overlaid, and alternates with an occassional "into the sperm". Given the heroin reference and time of release, I have always assumed this to be a reference to HIV transmission.

I'm convinced this song is about heroin.

It's cooked on a spoon, penetrated into the veins, transfusion into the blood, often makes those feel 'close to god" (just like Jesus' son), fly high and then (sometimes) die.
