IMO, this song is about inner torment and the fact that he's trying to move on from what it is that's burning him up inside, but it's useless, and he's left to his torment.

Adding onto what ninpunk says... that he knows he could of prevented what he has become, and regrets some of this mistakes he has made and now he's empty and cold with really no reason to live and he sees himself as that worthless, emotionally dead being everytime he looks at himself.
