This seems to be about anxiety+depression and/or drug withdrawals.

I absolutely love Lou Reed's singing in this song, you can feel the desperation/anxiety in his voice, and it fits the lyrics perfectly.

I love this. Perfect b side to heroin. Tough,gritty as only lou reed can. Proves that lou simply tells it how it is and does not glamorise. Street level visions of biblical proportions and standout on a great album.
