I think this song is based on the Paradise Lost story. The story of the Devil. Mainly from the "Fallen from grace" bits give that off. That and the images of war death.

hmm no i dont think its about paradise lost, i think about a group of soldiers walking back home after a battle, they are wounded and tired feel betrayed and lost, they cant see meaning anymore

I think it's about someone who's been used, and now the person is only going to do it to someone else. The narrator of the song is addressing this person and telling them how despicable they are, that this person is only out to use people.
The "Fallen from grace / Help me through" part suggests that she's tired of this persons games and selfishness and knows they're only going to do it again, but somewhere deep down she still wants to trust this person and wants to believe that they're not just gonna use everyone.
I think the song represents an internal struggle of this type... as in a thought process like this: "What a cruel dispicable little.. I don't see why anyone would want to associate with them, they're only gonna get screwed over. But I feel like crap now... I know he could make me feel better if they wanted to... Maybe I should-- No. I know this little game of his, and now he's only gonna do it to that other girl..."
Now don't get me wrong, it doesn't necessarily have to be about a relationship. It could be a close friend or even just a friend of a friend.. Anyone who could potentially screw you over at any given time.

I can see the Paradise Lost angle there, but its been a bit since I read it, so I can't give specific examples.
