I think it is about a woman whose whole identity is her marriage. She has only this one dream of starting a family but she can't pursue it. She doesn't seem to have the best relationship with her husband either, plus her fear of being alone may have made her marry someone who isn't right for her. Either way they have communication issues, he is expecting her to give her a family or at least she thinks he is. Somehow I think that it is all in her head and that he really would love her despite her being able to "bear him a son" or not.
