Tyler knew his grandfather would pass before this song would get produced, and he is sorry for not visiting him because also fell victim to alzheimer's, making him unable to recognize Tyler.

The day he passed, it is sad to even imagine this, Tyler recorded this section of the song just so he could remark how, maybe in heaven, they'll meet each other again and have lunch.

I believe it’s about a final goodbye to his grandfather. When he says “My middle name my good bye” he was his middle name is his grandfathers first name.
