I think this song it's about a relationship (can be romantic or platonic) that's starting to derail. And the guy is both desperate to hold on and trying to understand what the hell is going on with the person he holds dear. But unfortunately he gets no answer from the said person.
Tired of suffering from his own fears and assumptions in silence, he finally asks: "Is it worth it now?" as a final note to himself and/or to his friend. "Is it really worth saving this relationship at this point?"
