He's singing about the struggles of being LGBT and wanting to be accepted by God. He sometimes feel like his heart is 'mistaken' and wonders how can he still go to Heaven without changing who he is
This song deals with the internal identity struggle of LGBTQ+ people. Society, for a long time, has preached that gay people are sinners and that homosexuality is wrong. Internalized homophobia is a thing that most gay people deal with. It's not wanting to turn your back on your beliefs or roots, but not wanting to or even being able to change yourself fundamentally.
In addition to this, 'Heaven' can have more than one meaning. Heaven is a perfect place. Traditionally, this perfect place is somewhere humans can go after they die, so long as they either are not sinners or have atoned for any sins they may have committed. The alternative is hell. However, a "perfect place" can be defined differently. Perhaps, in this song, being with the one you truly love is Heaven.
In a roundabout way, Troye does a really amazing job of portraying the struggle between what you have been taught and how you truly feel. What is Heaven to you? And if you have to sacrifice an essential piece of yourself in order to reach it, is it worth it?
Troye himself confirmed that the line 'counting to 15' is a double reference. Troye was 15 when he came out for the first time. He also counts to 15 when he feels anxious as a coping method.

As lourology said this song is open to more interpretation than just the struggle of being from the LGBT community. To me this song is about not choosing something at the expense of your own well being and true self, would it matter if you were "normal"/ unnoticeable/ part of heaven etc. if you had to shut a part of yourself?
