This is a tricky one and I'm not sure I have an answer. Like many FR songs there are undertones of both love and drug use and I think an argument could be made for either one. There are literal references to both scenarios, such as continuously referring to "her" and "she" and references to his heart, which speak of love, but I think the more convincing argument is that he is referring to drug use, specifically an intravenous drug, such as literally saying "she's an uncut drug" and finding the "vein and the pulse" (scarlet "O" to me could be the blood entering a syringe or the scar left afterwards).
Perhaps the point is that love itself is like a drug, or somewhat vice versa that heavy drug use has deep repercussions that reach the depths of your heart and soul just like love. Regardless of the scenario, the author is clearly suffering with his inability to separate himself from the lover/drug or drug/lover.
