I think this song is basically when you lie about your pain and you pretend that you're stronger than you are. Later it starts to become devastating hiding your pain and she started to mention how if it was physical it would should and if it was spiritual she wouldn't know because she's hurting and burried her emotions deep inside and she doesn't know what to do with herself.
I'm truly not sure this is just what I think.
