Toss up between whether it is about being in love with someone, or in love with sex from someone in particular.
The first verse seems to suggest it is about being in love with someone (and this suits the music video message good). However, the remainder of the lyrics seem to suggest that the "sugar" he wants is purely sexual. The lyrics I think are a lot "dirtier" than otherwise suggested by the music video.

The whole ‘red velvet’ just seems like a metaphor for a vajajay. Not periods, not red hair...and it seems like he is referring to liking going down on whomever it is he’s singing about.
