I think it's either about someone who pretends to be happy, but isn't actually happy at all, or someone who pretends to be nice but is hiding a knife behind their back, ready to strike.
I think the smiler is the singer's assailant. not the singer. There's a key line which gets hidden in semantic games - it should be read as "(I) surrender will (noun): I am before you" (so no, it's not a pun on a certain ex-rapper-turned-broadcaster!)
