Lyrics don't quite seem right. They repeat the chorus (First verse) here more than once. Anyway, this song is about a guy who knows he's doomed; he's saying his last goodbyes to his friend Casey. It's also possible he's staying outside to fight some enemy and allow Casey some time to get away from things.

I sort of imagined this was a play on the age old phrase "Katy bar the door" meaning to be careful with danger ahead. And so the protagonist is recounting all of these dangerous times he's managed to pull through, but this time he's getting his affairs in order, sure that this one is too perilous to pull through.
