As of recent events in my life, I think I finally understand the meaning of this one.

For one, I'd like to point out that "Black Widow," another song from the Melophobia album, also uses a spider to represent a negative relationship. Obviously, in this song, there is some sort of connection between the writer and another person that did some not-so-nice things to him. By this connection, we can infer that the spiders in this song represent a thought that plagues the writer's mind and won't leave him alone, as is represented in "Black Widow."

I guess I need to talk about my personal experience in order for this to make sense. I've been suffering from a lot of self-doubt recently, and that includes blaming myself for every little thing that happens. One of my close friends got mad at me recently, and though they say they're fine and just need space, I still blame myself for it.

"I am the one you left for dead / ...You are the bullet in my head" means that this person has abandoned the writer when he has hit rock bottom, and she was the cause of this. "As you stand over my grave / Tell me it's okay / You pause the lips upon my face" probably means that though this person has figuratively "killed" him, the person tells him that she's fine and still kisses him, maybe as a good-bye or out of pity.

I think the next part is pretty self-explanatory, but either he feels super good around this person or he feels downright horrible. He feels like he is losing his mind waiting for answers to his questions.

"I think your mother wants me dead / ...I am a schizophrenic man." Schizophrenia is a mental illness where you have irrational fears of things that are not true or non-existent. This piece might mean that he feels so bad that he believes her mother might even want him dead, as well. "I wouldn't change you if I could" probably means that he doesn't hate the person for what happened and still admires them.

The chorus, I believe, relates to how his mind is poisoned with thoughts. "It all works out in time / You know I'm going to be all right" seems to be a promise that even though he is so low at the moment, he still believes he will get over it. "You may take my eyes, but baby, I'm not blind" could mean that although he can't stop thinking about her [all he can think about (see) so she "took his eyes," so to speak] but he's not blind to his other thoughts - his happier thoughts.

I'm entirely sure if this is accurate or understandable, but this is how I would interpret it.

I think this song, like many others, has to deal with Matt and his addiction to drugs. The "spiders" in this case, represents may represent the negative thoughts that wander in his mind, hence the title "Spiderhead". With this in mind, we can move onto the first verse which begins with the lines "I am the one you left for dead", I believe that this statement is the introduction to his addiction and how his constant abuse has left him for dead, in his grave, and "pauses" the lips on his face that feeds his addiction. The second verse consists of the contrast between heaven and hell, I think that the reason why he is either in heaven or hell is because though he may try and escape the reality that causes him pain, once he is alone in his own mind he is confronted by the spiders that haunt him whether he is high or not. All this time he believes he can seek the truth, and find the answers through his escape from reality into a new dimension, but once again he waits and waits to no avail of truth. The chorus repeats the line "You may take my eyes but baby im not blind", despite his sense of reality being masked by his addiction, he still sees the world for what it is, a constant reminder of suffering. The song ending with "You know im gonna be alright" is such a powerful line because to me when analyzing this I used my experience of drug usage to help guide me through these lyrics, its so comforting to sing along to the line saying it'll be alright. Though theres tough times, theres always a silver lining. This is what this song means to me. :)
