The speaker is talking from a state of depression, true depression, that you can't just will yourself out of. Its a state in which nothing matters, because the horizon is laden with defeating problems that no one else seems to care about. Is it a solution to take drugs (antidepressants), or does that make the speaker not care as well? Even when distractions arise (sex), they are only temporary highs before the everyday mundane things (saddest little cloth, water etc) remind and connect you back to your feelings of forlorn. Even your feelings lack that which you once felt, lack substance, and possibly describes the inner world, or "heart" or "soul".
Not sure about the first 2/3 of the song but I think the last bit might reveal what the song is about. It's about the world being such a terrible place once you really open your eyes and look at it objectively. There are people starving and dying all over the world. The environment is being destroyed. A tiny percentage of people hold the majority of the world's wealth. The list goes on and on.
Now, you could be "happy" if you just ignored all of this pain, suffering, evil, etc, but then you would be "in the dark." The speaker says that people who choose to live this way are "faint of heart" because they can't or don't want to see the truth.
Finally, the speaker offers to "leave you with an itty bitty hole" "if you give me your eye." If you choose to see the true state of the world that the speaker wants to show you, you will be left with the burden of knowing, hence less happy. However, you can always look away and probably forget after a while, but the speaker has accepted the truth for a while and doesn't have the ability and/or the desire to forget the truth. This explains why the hole you would be left with is "nothing like the one inside [the speaker's] soul."
