I think this song is about being depressed and passive-aggressive, and that the singer describes how to put up with this in a way which shows a lot of patience from his side, while barely managing to take it anymore.
This could be a result by long-time mistreatment from a girl, because what hits me is that it could be a description of almost any failed relationship with a girl (or a guy). That's why it is important not to lose yourself completely in another person. This guy has had enough of listening to others so-called "advices". He seems to understand now (finally) that he never had a problem until you came his way and made it so, and now he feels a little lost and angry for making him stick with you all this time. This is his moment to finally consider new possibilities and breaking free.
I think you need to keep this song in context with the album as the other songs talk about how he is openly gay and HIV Positive. I agree with you though that this song is about relationships and people giving him advice. What I'd really like to figure out is what does "You could be laughing 65, [63, and 25] percent more of the time mean? I wonder if this some reference to being HIV+?

@QXBear: I took it as a self-effacing joke. Instead of staying, "Stick with me and we'll have laughs all the time!" he's like, "Well, maybe 65% of the time. Or... 63%. Ok, maybe only 25%."

I think this song is about a guy telling the person who loves him that they would be much happier with someone else and that he can’t make this person happy. He's trying to understand why this person loves him because he can’t fathom why they would.
He lists all of the reasons he believes the person shouldn't love him: he's arrogant and narcissistic ("I am the greatest motherfucker that you're ever going to meet") but at the same time, that arrogance stems from a deeper feeling of inferiority.
He's says that the person must not know him very well because if he/she did know him they wouldn't love him (he's angry, an underdog, feels sorry for himself, isn’t good at give and take or relationships, thinks only about himself etc.)
He states that the person thinks he must feel bad about this stuff but the guy says that he didn’t feel bad about any of it until he developed feelings for this person and started seeing himself from another person’s eyes. He really DOES feel bad about it and he’s angry at the person for making him CARE. He’s saying that this person has made him (care) love them back, which he doesn’t really want to admit to himself. He’s in denial about feeling bad about not being good enough for this person, disliking himself, and about how much he really does love this person back. It’s easier for him to tell himself that he’s fine with being how he is and not letting himself care, but deep down he knows he does and he’s mad at the other person for ruining his ability to deny and ignore these things about himself.

...is the guy saying that the person who loves him tells him that he needs to learn to love himself (because they see through his arrogance and see his inferiority complex underneath) but he calls the person Dr. Frankenstein because things that the person has tried to change about him are the very things that contribute to his inferiority complex. He tells them to make up their mind. Should he love himself or should he change?
Throughout the song he tells the person to go ahead and love him if they’re going to but to remember they could be laughing 65% more of the time. He’s saying that this person won’t stop loving him no matter what he tells them about himself but he tells them to remember that they would be happier 65% of the time if they were with someone else. He feels bad that being who he is takes away some of this person’s (who he cares so much about) happiness and he wants them to leave him because he wants them to be happy.
At the end of the song the perecentage decreases. He’s goes from 65% to 63% and then all the way down to 25% by the end of the song. I think this could be interpreted as the guy admitting to himself that he doesn’t want the person to go or to stop loving him, that he loves them, that he’s not as arrogant or unfeeling as likes to convince himself he is.

My favorite line of this song's lyrics is the dig that Grant directs to our society's rejection of the validity of gay love/unions where it says:
"So go ahead and love me, while is still a crime..."
The song begins in a fun way because it could seem like a very self confident person (knowing himself) But perhaps only within his own confined boudaries as at least the music video shows a loner person. Whatever reason for that (primarily one self but others can also have kind of added to it), he has the challenges of entering society as a more normal person.
There are many things I do not understand and I do not have complete overview of the song (yet) but this one is fun: "i should not be attracted to males but you said that i should learn to love myself make up your mind, doctor frankenstein" it depicts the that there are several kinds of love at least on this earthly plane because of course we should love ourselves even though we are males and hence bodily love is not all as men can easily get lost to think (mind love is not either though, at least not on this plane)

This song is humorous take on the bullshit artists and self-important that live with us, it is encapsulated in the lines. "I am usually only waiting for you to stop talking, so that I can, Concerning two-way streets, I have to say I am not a fan, But I am the greatest mother fucker you are ever gonna meet," The following lines, "From the top of my head to the tips of my feet, ... you could be laughing 65% more of the time," could be bullshit "percentage" talk and self-importance these shitbags and know-it-alls vomit in when you talk to them. "..love me while it's still a crime," This could be a reference to shacking up when gay, There is some other lyrics where Grant sings about "having the nerve to make me feel" and "I should have practiced my skills, I should not be attracted to males make up your mind Dr. Frankenstein," shows that angry edge these people show when confronted, The closing lyrics, "You could be laughing 65% more of the time, -You could be laughing 63% more of the time, -You could be laughing 25% more of the time," Is a clue to detect these backpedaling and lying people.
Basically it is a PSA & tribute about these people that we share the Earth with.
If any one has cared enough about the lyrics they would learn about John Grant instead of applying the words to their own life as “the meaning” of what John wrote, they would have researched the artist, as much as possible and as available, through interviews that were published and are readily available simply by putting “John Grant music artist” in the search bar of any decent browser. John Grant brilliantly shares his life publicly, his trial and tribulatinons, sucesses and joy with any situation and by doing so affords others the chance to see the other side of said situations and an alternative point of reference to gain self respect. Read about the man and you will fully understand what and where he is coming from. I did, I do and as sappy as this may sound, knowing that someone else felt exactly the way I did and do concerning matters of the heart saved my life.
