I think this is just about being selfish, and wishing you could be more nice, caring and genuine but not being able to. It's self-conflifction; "I wish there was something I could do, I hate myself more than I hate you" and there being a level of care, but not enough for them to sacrifice their own selfishness for "I can't be real, and I can't be nice; there is nothing in this world I would sacrifice"
