Well, this is obviously a story song, and I think it's about a sick sister. This guy probably didn't have parents, and his sister played the role of his mother, and they probably had other siblings too. She sacrificed everything she had to make sure that her siblings got everything they needed and were never left wanting for something. And I guess in the process of that she sacrificed her health and she got sick, but she never told anyone. Eventually her health got bad enough and she collapsed, where the guy found her at the bottom of the stairs. I assumed that they were grown people by this point because that would explain why she "didn't care" when he was taking her to the hospital, because he was settled and doing well.
In the second verse, I think he's trying to come to terms with his sister's sickness because it hit him very hard. He talks about how scared he is of losing his sister, and how he thinks he would have dealt with it better had he been more prepared. He's shaking and he's probably crying because the thought of living without his sister just terrifies him so much.
The third verse and bridge is probably a continuation of his fears and nightmares, because he feels unsafe without his sister. And the devils could be facing her death and moving on with his life without her. And he's in the hospital but he can't stand it there because it's too much for him, because every time he sees his sister's paling face he knows she's fading and her health is weakening.
I'm not sure about the final chorus's lines but I think he feels betrayed when he sees she's so cool about her impending death, because he thinks that she wants to leave him and she doesn't love him. He goes delusional with grief.
I'm really not sure about the last lines of the song, does anyone else have any views?
