I think this song is about a recovering addict and/or alcoholic trying to rescue a lover from addiction, or about the predicament of their co-dependent relationship. It sounds like they were together when they were both abusing substances, and now the protagonist has recovered and knows that her own well-being has to trump the relationship. So she will help the lover one last time, but then she must move on.

It's a beautiful song. She's fabulously talented and should be more well-known.
