I think overall, this song is about how at the end of the day, all we really have is ourselves. And how nothing in the world can ever make you feel more alone than that.
The sense of inadequacy you feel whilst watching those you value so much feel such satisfaction in your mere absence is heartbreaking: "And my friends are out getting drunk somewhere without me."
There is a beautiful plea for acceptance, "Oh will you open up the door?" But I believe this song strongly portrays the notion that we are all just the same, but this equality is only acknowledged "until we all fall down like water".
