I love the horn sections at the start of the song and as an interlude within the song too.
What does it mean? To me the only thing I can interpret it as being is "depression". That is the "enemy" that the narrator continually relates to is themselves (and most likely the guise that depression takes in their life).
Depression is a bit like a thorn, which is used the metaphor. In that similar to a thorn it hurts, takes hold and is hard to take out (get rid of), and if left untreated the horn can infect the wound (much like how depression can worsen if not managed early on).
