I think this song is about hypocrisy/watching someone lose their religion. When he says "I found you holding a nail and a fragile piece of paper What a lovely plot, nail it to a door" I am imaging some religious bigot nailing up bible verses in a sense to spread the gospel.
Then when he says "Two years later with a bag and a bowl. What a firm grip. Whats true to you?" I think two years later he sees this person has changed completely, doing drugs (most likely marijuana "bag and a bowl") He is sort of laughing at them like "what a firm grip" the religion had on you .
The rest of the song, I don't know. Just goes onto what is happening in his life, "Cancer in my heart" could be that his heart has grown heavy with age and stress.
