I think this song is about the fear of romantic relationships turning bitter.

There's consistent imagery of water and drowning, "rain, umbrella, drinking, lung, thirsty, breathing" and repeatedly at the end "Don't sink". This suggests that the person here is afraid that the commitment of love will swallow them whole and drown out everything else. The line "they don't make no love they just go to war" displays that. The love they once had has been replaced by fighting and there's nothing but conflict left. "They're just breathing fire, pulls his teeth, pulls her hair" all show the frustration and anger of " the man and the woman next door", presumably a couple stuck in a bad marriage. So in an effort to protect themselves, they don't get, "Heart Heavy". They don't desire a romantic relationships because fear how it will end.

This idea is further supported by the line "I don't wanna make the same mistake". The mistake of romance. The fear that the sweet, young love won't last forever and in the end you'll just hate each other. The person is afraid of this perceived reality. So they insist that they "Don't sink because my heart's not heavy". They insist that they'll never fall prey to the trap of romance because they decide to never pursue a romance for themselves.
