On the official Alter Bridge blog entry for AB III, Myles is quoted as saying, "The lyric was inspired by the idea that there will come a day when we all must say goodbye to the ones we love and hold dear in our hearts. Basically, I asked myself what would I want to say in that final moment, that last goodbye?”
He's also said, although I can't find the source, that the song is about a man losing his wife to disease.
This is an extremely touching song, right on par with In Loving Memory.

What an absolutely amazing and powerful song...Tremonti and Kennedy really know how to tug and pull at your emotions.
All I know is that I was able to share this song with someone who meant so very much to me. It may save the relationship we had, but it helped her understand the relationship we could still have. It may not be the one I want, but it's the one I'll gladly accept rather than having her erased entirely from my life.
