These lyrics are said by someone who put their heart out there for someone. One's chest always feels heavy and always hurts when thinking of a past love.

This song is about UNREQUITED LOVE. It's about the unbearable pain one feels when they are in love with someone who doesn't feel the same way.

I think it's about dying. Either natural or suicide... And the singer is telling someone they love goodbye. They always say "I loved you" not "I love you". And you don't say "I loved you" unless you're going somewhere. And judging by the sadness of the song... It sounds like someone who's really depressed saying goodbye to someone they loved.
