What I got from this song is that it's about his mother or someone really close that has gotten cancer and is dying. What's happening is he is asking God to let him speak to her one last time. Then his prayer has been answered and she is saying her last words, but he is terrified that he can't hear her last words. This is one of my favorite Christian songs :)
