OTB is, presumably, Off-Track Betting, and the song is a continuation of the "Chips Ahoy!" story.
I think that this song is a come down to chips ahoy, but I've also been speculating that Chips Ahoy, Both Crosses, Yeah Sapphire, and The Weekenders are all about the same metaphorical girl.. Anyone have any thoughts on this?

I saw them last night in SF. The song was introduced as: "This song is about the exact same couple" as in Chips Ahoy.
i think this is the come-down to Chips Ahoy. and i think Craig is deliberately fucking with the heads of those of us who try and construct a linear narrative out of THS songs. i thought we dealt with the horses and then moved on to some kind of murder mystery, no? apparently not. i guess this kind of looking back over it post-breakup. i don't get it though. who's "they" in the second line? (i also briefly thought OTB was ODB, as in the late great Ol' Dirty Bastard.)

Not sure about Both Crosses, but Sapphire's almost certainly the girl in Chips Ahoy (and consequently here).

I was hoping the line "You could say our paths have crossed before" was "You could say I passed a cross before". Anything to make this album more interesting. It's a damn shame too, a riff this monstrous and a sequel to chips ahoy deserves better.

to me this song reminds me of home. my (ex) best friend was a huge hold steady fan and we would always listen to them when we were together which was often spending all weekend together. the song has eerie coincidences in it, like "the whole weird thing with the horses" SO many memories of horses with her as she lived on a farm. another one is the "i remember the OTB" which is also the bar where i live. and the many parties we've had at the reservoir. but all in all i love this song and it just makes me happy.
