i think this could be about a guy who lost a friend to suicide, and he beats up another 'friend' that kinda pushed his friend over the edge and he ended up killing himself. i could be way wrong but that's just my interpretation. it could make sense because the first part of the song is talking about losing his friend then the second part is talking to the guy he's beating up because he's mad, which explains the bloody knuckles and why he would need a doctor and would be scared
