maybe Hes talking about shooting up listening to foreigner and being a foreigner himself...

Well, that might be it, I might be crazy to think that he would be "talking" to these things but its kinda like havin an argument with yourself or maybe an argument with your addiction and your conscience is saying "no!" but the drugs win...

This is a great song. I'm not sure what it's about, but my guess would be shooting up just because of the needle hitting the groove reference.
I also know that Nick and Jamie are 'foreigners' to the US where they are doing most of their touring for this album. Besides that, they might be Foreigner fans since one of their old Unicorns songs was titled "Hot Blooded". Or maybe Nick feels like he can relate to the band Foreigner because of the number of band mates that keep coming in and out of the Islands.

I just assumed the needle was a reference to a record. Maybe don't "drop" the needle on the groove? I assumed Nick was referring to listening to a Foreigner record, playing off the fact that Nick is a Canadian living in New York City.
Referencing a record makes a lot more sense. I initially interpreted the line as, "Drive the needle in the groove" which I thought was drug related.
This song is about contracting AIDS from a needle.
it could go both ways. simply being a record on a player or heroin related but necessarily just that. as a former heroin addict myself, i don't fall on the side of it being about shooting up. <shrug>
