A "die slow" is a nickname for a person with AIDS. A repeated line in the song is that no one knows she has it either. I think it's about people who contract HIV but are unable to stop fucking other people at parties out of spite. She doesn't care if she gives it to guys or girls, as long as she spreads her misery. There's no curing yourself; "how do you come back?"

Amazing song, but absolutely brutal live!
I was fortunate enough to see them a while back. I, in an idiot-savant flash of brilliance, decided to be in the pit. A mere chorus into this song I found myself with a slight nosebleed, missing a shoe.
