This is probably about a cocaine or crack relapse. The singer goes back to 'some places that I know' and gets high to see if its still the same. Then the song switches to the 3rd person.
The 'winds that tear him up' are painful thoughts that 'breeze out to pasture' under the influence of the drugs, while they 'stay close and comfort' the singer.
He is high, flowing, mellow. he never wants to leave this artificıal heaven.
Cocaine and/or crack are suggested by: 'a little out of sleep', 'frozen teeth' (the numbing effects of coke), and 'melted lıps' (a hot crack pıpe can serıously burn your lips). he knows he looks like shit, but doesn't care - he feels great, peace and yearning. For now.
The next lines give more bliss images, plus the reason for the relapse: the straight world is full of shit, and he will sacrifice it all (kill everything) to get away from the 'hollow'.
Again, the sweet worlds of angels are the blissful realms of the high. Which is eternal, if you keep going back for more. Or, at least, you wish it was. And you don't want to live without it. 'Your altar perfuming up my life'.
