this song is pretty obvious. Its about a girl who is acting weird all of a sudden and the guy suspects its to do with him and that he has done something wrong but he doesn't know what.

Perfectly good heart = getting your heart broken and asking why, what did i do to your heart = guy wondering what he did wrong to get the girl upset with him and wondering how he can make it up to her.
