I love that song. I think it's about a person being in love with somebody even after an ugly breakup that they know was their own fault, for what could be any number of reasons. But no matter how stubborn they are in NOT admitting they were wrong and that they still have feelings for that person, they're still in love with them. And they realize that they may have hurt that person at first, but that person will move on eventually. Really in the end it's their OWN heart that they broke. (i.e: "when you break your own heart, it will never mend")

There's more to it than that depending on how you look at it, but that's the main things I think of when I listen to it.

Going off of the previous commentor's interpretation; i think its about a person who leaves his girlfriend/wife/significant other/etc. after a big argument with her. Then after a while he realizes that he still loves her, but before he can tell her she's hit by a car and dies.
