A little soul-searching. The first part, he's just himself, not the superstar JT, helping a friend. The next part deals with the death of John Belushi, his good friend, who overdosed. JT nearly died 5 times from overdoses, and contemplates how it could have been him. The fortune and fame acknowledges the icon that he's become, and the lifetime fan following that he has. The last part is a reference to his 2nd wife (at the time) and how they needed to work through their problems.
I think it's about touring and finding a way to reconcile himself to playing the same songs over and over for the people who love them. I think all famous musicians have to struggle with this. Some of them come to hate the audience. For JT, he learned to see this as his job, the reason he was born, to keep playing the songs we love, live.
