So, here's what it sounds like to me. Whatever he did (or failed to do) caused him to feel empty inside (line 2), as though there was a hole inside of him (line 1). It sounds like he gave into the empty feeling rather than trying to fix the problem ("...fell inside myself"). He feels like it's a disease and wants more out of life than the emptiness he feels (Chorus). He's trying to prevent his life from collapsing ("I push to keep...") but can't seem to fill the emptiness within himself ("Trying though..."). etc. Cool song, good lyrics. Any other interpretations?
In this song the disease referred to that is desired to break free from is probably sin. For me though, It has another meaning and it has to do with my illness that I personally have and how I want to get cured or overcome it. But I can't so I am doing my best to fight it and working towards keeping it under control and maintained.
